i dunno if you want hooks because you have codes that you want to use together, or if you are writing a code, but i use this to correct hook issues:

so basically if you have two codes that have the same hooks, you can change one to 0x004bc8a0 and one to 0x004bc8a4 and change the J in the hook to a JAL like so:

from: 0x0a400000 to: 0x0e400000

and it should work.. i dont know if this helps you or not, because you didn't describe exactly why you need to know all the hooks.. this is a good code to use though, if two codes you are using have the same subroutine.
